Using git bisect the "void QCocoaMenu::insertNative(QCocoaMenuItem *,
QCocoaMenuItem *) Menu item is already in a menu, remove it from the other menu
first before inserting" messages first appear with commit 5025bc2 for
1.99.1-431-g5025bc2:

This makes sense as the Qt UI components were touched with this commit.
What makes less sense (to me) is the date of 2013-12-21 listed with commit
5025bc2. The previous commit 15e78de for 1.99.1-430-g15e78de has the date
2014-10-31 while the subsequent commit e41f088 for 1.99.1-432-ge41f088 has the
date 2014-11-04. Guess I need to read up on what the Date field of the git
show report really means.
