There are enough issues with this patch that I'm backing out the change and
reopening the bug.

1) The UI guidelines for three of the four major desktop environments -
Windows, GNOME, and KDE - all recommend that the window title *not* begin with
the application name; that means that the truncated titles that tend to show up
in taskbars might show only the application name, so you have N
indistinguishable taskbar items for the app if you have N windows open:

http://developer.kde.org/documentation/standards/kde/style/basics/windows.html

"The application window should be labelled with the name of the document in the
window, and the application name, separated by a hyphen. For example,
"www.kde.org - Konqueror", "StyleGuide.ps - KGhostView", or "(untitled) -
KEdit"."

    http://library.gnome.org/devel/hig-book/2.30/windows-primary.html.en

"While document names are most pertinent to users, we understand that
application developers may want to increase recognition of their application.
If you plan to include your application's name in the title of a primary
window, use the following format: Document Name - Application Name. This will
ensure that the document name appears in limited space situations such as the
system window list."

    http://msdn.microsoft.com/en-us/library/aa511446.aspx

"When choosing window titles, consider the title's appearance on the taskbar:

Optimize titles for display on the taskbar by concisely placing the
distinguishing information first. ..."

2) The change doesn't affect live-capture windows or "idle" windows (no capture
in progress and no file open).

3) It's not updating the title bar as soon as you change the check box setting.
