Or if you're monitoring a port that's replicating another port's traffic, you're physically inserted "in-line" with a port that contains the traffic you want to monitor, etc.

This kind of question and a description of monitoring techniques in general would be great to add to the Wiki =)

You can't, unless you are on a hubbed LAN, and the machine you want is
on the same LAN.
